What you need
If you are looking to refer someone to Goodwin Hospice services, please include the following documentation with all hospice referrals.
- Patient demographic sheet with patient’s full name, address where they receive hospice services and insurance information
- Referring physician’s name and phone number
- Physician order for needed hospice services
- Patient’s recent physician’s note, history and physical or hospital paperwork
